Speeding Tickets: Is this a legal ticket?

My question involves a traffic ticket from the state of: Utah



I was pulled over and was given a speeding ticket. The officer was curtious, but said that he was out of ticket paper. The officer gave me a page of a memo note pad as a "ticket." On the page is the case #, # for the court, address of the court and the speed. The officer asked me to sign some random paper that he came up with, and on the paper was my speed. That was his copy. He wrote my address and phone number on this paper. I asked him if signing the paper was an admission of guilt and he said no. It seemed strange that he would give me a ticket that way at the time, but looking back on it now, it seems like a huge violation of my rights. No info about the cops name, department, etc. I am planning on going to court. What do I need to do to make sure this is dismissed?
