Federal Taxes: Form 8962 with Part Year Minimum Essential Coverage

I can't find anything explaining how to deal with line 69 of Form 1040 or Form 8962. For the first part of the year I had insurance through my employer, with the premiums paid by the employer. Then I had COBRA for 6 months and I paid the premiums. Then when the unemployment expired I had a Medicaid plan from the state - "minimum essential coverage." This state does not send a Form 1095-A for minimum essential coverage. When I filled out Part 1 of Form 8962, my total annual income was slightly above 400% of the poverty level. So according to that I am not eligible for a tax credit. But for two months the state qualified me for the "minimum essential coverage" - Medicaid. I am lost about what to do about Form 8962, how to fill it out, whether I need to fill it out if Medicaid isn't a "tax credit" and whether I have to pay anything back to the federal government if I had two months of a Medicaid plan but, for the entire year taken as a whole, was above 400% of the poverty level.
