I was born in 1956. When my parents applied for my social security card they used my stepfather's last name which is not the last name on my birth certificate. I have been using the last name on my social security card for approximately 50 years and never had a problem except when I joined the military in 1973. The military required me to bring a document from my high school confirming I was the same person that my birth certificate and social security card showed. I still have the document from school. the military allowed me to continue using the name on my S.S card and informed me that I was now legally the name on my S.S card. Now I am unable to renew my state ID/drivers license because of the last names not matching on my birth cert. and S.S card. the people at the DMV told me that I needed a court order verifying my identity. I was told by the clerk of the court, I needed to submit a adult name change form and apply for a amended birth certificate. can I get a document quickly from the S.S. Administration that would allow me to renew my state ID card?
