Hearings and Trials: What is the Proper Process of Requesting a Trial by Written Declaration

My question involves traffic court in the State of: California



Hello, I have recently just gotten my first speeding ticket and would like to request a trial by written declaration as it does not state that my appearance is mandatory. I am confused however with exactly how I am suppose to do this. On one hand, it seems like I am suppose to fill out this official form:

make my statement, submit evidence all with my request as well as to post bail.



On another hand, I came across this site:

which also provides a form for a request for trial by written declaration. here they said that it is not necessary for me to provide statement, submit evidence etc as I am only requesting the Trial and that I don't need to post bail. They then also provide another form in case my request is denied under the grounds that I need to post bail first. I am also confused if I should include my citation or the courtesy notice that came in the mail.



Any light people can shed on the matter would be greatly appreciated. I would really like to know the proper way to do this.
